Why Your Skin Looks Different in Your 40s and 50s, and What Can Actually Help

You Don't Look Tired. Your Skin Has Just Started to Change.


You're not imagining it.


You've noticed it in photos, in mirrors, in the way the light catches your face at certain angles. Something has shifted. You don't look ill. You don't look dramatically different. But you look tired, even when you're not. Or a little gaunt. Or like your face has somehow lost its padding, its structure, the thing that used to make you look like you.


And the frustrating part is that you're doing everything right. You're sleeping. You're drinking water. You wear SPF, most of the time. You've got a good skincare routine. But none of it is quite touching whatever this is.


This is the conversation I have almost every single day in clinic.


And the reason no amount of skincare is fixing it is because what you're describing isn't a skincare problem. It's a collagen problem. A structural problem. A problem that starts happening quietly in our mid-20s and then accelerates, often dramatically, around perimenopause, to a point where clients sit across from me and say: "It's like something happened overnight."


It didn't happen overnight. But I understand why it feels that way.


What's actually going on


Collagen and elastin are the proteins that give skin its structure, firmness, and bounce. They're produced by cells called fibroblasts and they form the scaffolding that holds everything in place. From our mid-20s, that production starts to slow. By the time we reach perimenopause and menopause, falling oestrogen accelerates the decline significantly, and research shows women can lose around 30% of their skin's collagen in the first five years after menopause alone.


That's not a little. That's a third of your skin's structural foundation.


And when the scaffolding goes, everything above it starts to shift. Volume drops. Skin thins. The face loses its shape. Areas that used to look full start to look hollow. The jawline softens. The cheeks flatten. The temples hollow out. The neck changes. And all the moisturiser and filler in the world can't rebuild what your body has stopped producing.


What can begin to rebuild it is a biostimulator.


So what is a biostimulator, and why is it different?


This is the part where I need you to forget everything you think you know about injectables.


A biostimulator is not a filler. It doesn't add volume by putting something foreign into the space. It works by triggering your own body's collagen production, so that the results you see are built by you, from your own tissue, over time. The product itself eventually breaks down and is reabsorbed. What stays is yours.


This is why results from biostimulators look different to filler. They don't arrive all at once. They emerge gradually over weeks and months, getting better and better as your collagen builds. Which means instead of a sudden visible change that people around you can't quite place, you start to look steadily more like yourself. Refreshed. Rested. Like something good is happening to you, but nobody can pinpoint what.


That's not a side effect. That's exactly the point.


I recently completed advanced training in both Sculptra and Radiesse, the two most clinically established biostimulators available, and I want to explain what each one does, why I chose to train in both, and how they might help you.


Sculptra: for when your face has lost its shape


If you've looked in the mirror recently and noticed that your cheeks look flatter, your temples have hollowed, your jawline has softened, or there's a gauntness to your face that wasn't there five years ago, Sculptra is likely the treatment I'd be thinking about for you.


Sculptra is made from poly-L-lactic acid, a biocompatible, biodegradable material that has been used safely in medicine for decades in things like resorbable sutures. When it's injected, it triggers a controlled response from your body that recruits fibroblasts to the area and stimulates them to start producing new collagen. As the Sculptra gradually breaks down over time, the collagen your body has built in its place remains. You're left with your own tissue, your own structure, rebuilt.


The clinical evidence is substantial. A 2025 multicentre randomised controlled trial in 331 subjects found Sculptra achieved over 90% improvement in midfacial volume at twelve months, outperforming hyaluronic acid fillers. Earlier research showed a threefold increase in skin thickness that was sustained over two years. Results typically last in the region of twenty-four to thirty-six months.


For most clients, I recommend two to three sessions spaced around four to six weeks apart. This staged approach allows the collagen response to build gradually without overcorrection, and results continue to improve for up to six months after the final session. This isn't a treatment where you see everything at once and then watch it fade. It works the other way around: slowly building, then holding beautifully for a long time.


Sculptra works well for the temples, cheeks, mid-face, jawline, chin, and along the jaw. It can also be used on the body: the buttocks, inner arms, abdomen, and neck.


Radiesse: for when your skin has lost its quality


Radiesse is a different product and it solves a slightly different problem, though often the two work hand in hand.


If what you're noticing is less about lost volume and more about skin that has lost its quality, its firmness, its texture, areas where the skin feels thinner or crepey or has lost that taut, healthy look it used to have, Radiesse is often the treatment I'd reach for first.


Radiesse is made from calcium hydroxylapatite, a mineral that occurs naturally in the body in bones and teeth. When injected, the microspheres create a physical scaffold that directly activates fibroblasts and triggers regeneration of collagen, elastin, and proteoglycans, the proteins responsible for structure, elasticity, and hydration. Unlike Sculptra, Radiesse also provides some immediate improvement from the gel carrier, so you're not waiting entirely for the biological response to kick in.


The research is compelling. Radiesse is the most researched biostimulator in aesthetic medicine, backed by over 250 scientific publications. Clinical studies have shown it can boost collagen production dramatically and increase elastin by up to 96%. It carries a 98% patient satisfaction rate, and new research published in 2026 showed it also supports broader skin remodelling at a cellular level, including improvements in UV-related skin damage, which is particularly relevant for areas like the neck and chest that have accumulated years of sun exposure.


When used in a diluted form, hyperdiluted Radiesse is one of the best treatments I have for the neck, décolletage, hands, inner arms, and knees, areas where skin has thinned and lost quality rather than volume. Used in standard form, it works beautifully for facial contouring and skin quality improvement across the face. Results typically last twelve to eighteen months.


Most clients need one to two sessions, depending on the area and the degree of change they want to address.


Why I trained in both


Because your skin deserves the right tool, not just the only one available.


Sculptra and Radiesse work through slightly different mechanisms, have different longevity profiles, and suit different presentations. Having both available means I can genuinely build a treatment plan around what your skin actually needs rather than defaulting to what I happen to offer.


And more often than not, the answer isn't one or the other. It's a considered combination. Perhaps Sculptra to restore structure and volume across the face, alongside hyperdiluted Radiesse to improve skin quality in the neck. Or Radiesse to address immediate skin laxity while Sculptra builds longer-term structural support underneath. Layered thoughtfully, the two complement each other in a way that neither achieves alone.


They can also be combined with polynucleotides for skin repair and hydration, with medical-grade skincare to protect the collagen we're rebuilding, and where appropriate, with carefully placed filler for specific structural concerns. None of this is off-the-shelf. It's built around you.


What a treatment appointment looks like


Appointments for biostimulator treatments at South Hams Aesthetics are typically sixty to ninety minutes. That includes time at the start to talk through your concerns, review your health history, and agree on a treatment plan before anything else happens. I never go straight to treatment, and I never recommend something that doesn't genuinely make sense for you.


The treatment itself is carried out with topical anaesthetic cream applied beforehand, and most clients find it very well tolerated. Afterwards you should expect some swelling, redness, and possibly some bruising, which settles within a few days, and mild tenderness for up to a week. There's no significant downtime, though I'd suggest avoiding strenuous exercise and alcohol for twenty-four hours after treatment.


Results develop gradually. With Sculptra, the transformation builds over three to six months. With Radiesse you'll notice some early improvement, with deeper results continuing over the months that follow. Both treatments continue to look better as time goes on, and that gradual improvement is part of what makes them feel so natural.
